Intake Visit: A one-hour session to assess symptoms and history and begin a treatment plan. Follow-up Visits: Appointments will be scheduled for 30 to 60 minutes. To remain active, appointments are required every 3 months. Monthly appointments are required for to refill controlled substances. Office Policies: Before prescribing, I may require medical records and/or medical clearance from your primary care provider. A $100 fee applies for no-shows or cancellations within 48 hours. In our initial sessions, we will get to know each other. I want to understand your feelings and your treatment goals. We will review your mental and physical health history, and I will ask you many questions about significant life events and relationships. I understand that trust builds slowly, so if you have experienced trauma or find it difficult to open up, don't worry—we will proceed at your own pace. We will assess how you are doing in each session and discuss your next steps.
